Using spacers and washers

Working on cars for Mid-Amarica. Ordered wheels from John along with spacers and washers. I have never ran wheels reversed. Do the washers go against the axle head or body? Which way do the spacers go? Do they just float between the wheel and body or do I attach them to the body? I have not been able to find any info.
